How and where to file

Missouri accepts the application online, by phone, or on paper. A nursing home applicant must also submit the Aged, Blind, and Disabled Supplemental Form. Completed forms and later verification records may be uploaded online, mailed to the Family Support Division, faxed to 573-526-9400, or delivered to a local Family Support Division Resource Center.

Complete Records Means Complete

A screenshot, transaction history, or selected statement page may omit the owner, account number, statement period, balance, or transactions Missouri needs to verify eligibility. Missouri generally will not accept screenshots or statements with missing pages as a substitute for a complete financial statement. If the financial institution cannot provide a complete statement, it may need to provide a letter or another official record containing all of the required information that the statement would otherwise have shown.
